Surgery Scheduling
Who do we call to schedule?
Call surgery center direct at (262) 928-4336 between 8a.m. to 4:30 p.m.
Our scheduling system provides scheduling by CPT code. Preauthorization will need to be completed by physician office and a copy of the patient insurance card needs to be faxed to the surgery center no later than 48 hours prior to the day of surgery so we can complete the pre-registration process.
What insurance companies you are participating with?
We participate with most health insurance plans in the area, i.e. HealthEOS, Trilogy, Aetna, Humana, Anthem, WEA, WIDS, Unity Health, United Healthcare and WPS. Please see the list of accepted plans under the insurance contract section of our site or contact the facility directly with questions.
What type of equipment do you have and what type of cases can we schedule?
Admission criteria and pre-op testing requirements are provided in the "Patient Selection Criteria" policy under the Our Provider tab. Pre-op orders need to be faxed to the surgery center within 24 business hours of case scheduling. A copy of our Pre-op Orders can also be obtained under the Our Provider tab. Special equipment needs should be addressed at the time of scheduling and availability of needed equipment will be provided with schedule confirmation. Are Surgical Assist available?
You will need to provide your own first assistants. A second scrub tech can be provided when available.
How late can cases be scheduled?
The Operating Room is scheduled to perform procedures between the hours of 7:15 am and 3:15 pm. Cases will not be scheduled to conclude past 3:15 pm.
Does each provider need to be credentialed for the surgery center?
New credentialing will be required for all providers. A credentialing application can be obtained by contacting the center or downloading the application from our website. Please contact the facility administrator at (262) 928-6080 to gain access to our secure documents under credentialing documents section of our website. Providers cannot begin scheduling until credentialing has been approved by surgery center credentialing committee.
What are the options for scheduling cases?
Option 1:Request surgery within EPIC with a Case Request. Make sure you are providing all information on request. If you are requesting any specific items for the surgery please utilize the "Special Needs" section at the bottom of the request.
Option 2:If you are not on EPIC you can utilize our paper scheduling order located here. Make sure it is filled out in its entirety and fax the order to 262-953-8811.

Please fax us the scheduling form and insurance information, we will schedule and fax back or we can email confirmation back to the office scheduler same day if faxed prior to 4:30pm. All faxes received after 4:30pm will be scheduled next business day. Confirmation will be sent via fax or email as soon as the case is scheduled.

When you phone in we need the following:
- Date and Time of Procedure
- Patient First and Last name
- Date of Birth
- CPT Code and Ordered Procedure
- Surgeon
- Duration
- Diagnosis Codes
Then same day we will need the form filled out and faxed down to us along with demographics (address, phone numbers) and insurance information. Confirmation will be provided at the time of scheduling.
